Request Card Key

Use this form to request a card-key for an individual.

Please carefully review and acknowledge the below conditions to request access credentials to the facility.

Access card-keys are non-transferrable and MAY ONLY BE USED BY THE INDIVIDUAL TO WHOM THEY ARE ASSIGNED. Use by any other person is unlawful, and may be prosecuted as criminal trespass.

Lost or stolen keys must be reported at once to building management so that the lost key may be deactivated and replaced.

All activities within common areas and surrounding the facility are under video surveillance. All activities are recorded at all times. Video recordings may be utilized along with access records consistent with ongoing security efforts.

Access may be restricted or suspended at the sole discretion of building management for any reason including but not limited to utility power failure, construction and renovation, or other incidents as deemed by management.

No door which otherwise closes automatically shall be allowed to be propped open at any time.

No attempt shall be made to circumvent security systems, or attempt access to areas other than those that I am specifically authorized.

By my acceptance of the above conditions and my typed signature I hereby understand and agree to the stipulations set forth in this document. I further understand that failure to adhere to the conditions above may result in the loss of access privilege to the facility, civil legal action and/or criminal prosecution.
